The Lambton Kent District School Board (LKDSB) will be initiating a Request for Supplier Qualification (RFSQ) for General Contractors that can safely construct Blue Coast District School, to be located at 6314 Townsend Line, Forest, Ontario, in a timely, cost-effective manner while insuring the utmost in craftsmanship.

All General Contractors that want to be considered for this project shall submit a response to this RFSQ, regardless of whether the General Contractor has been approved previously by the LKDSB Contractor Qualification Process. 

All subcontractors that want to be considered for this project shall be included on each General Contractor's RFSQ submission package.  Each General Contractor is responsible for their unique list of subcontractors.  

Any subcontractor not listed in the General Contractor RFSQ submission will not be eligible to submit pricing in the ITT stage.

Regulatory Requirements:              

 This Project is subject to the following trade agreements:

• Canada-European Union Comprehensive and Economic Trade Agreement
• Canadian Free Trade Agreement
• Comprehensive and Progressive Agreement for Trans-Pacific Partnership   

The submissions must be completed in the English language.

Conditions of Participation:

Province of Ontario Procurement Restriction Policy:
Respondents that have a head office in the United States and less than 250 full-time employees in Canada will be disqualified, in accordance with the Province of Ontario Procurement Restriction Policy. 

WSIB Safety Rating:
Only Respondents that have a 5-year average allowed lost-time injury rate (per 100 workers) below 2 and an allowed no lost-time injury rate (per 100 workers) below 4, as measured by the Ontario WSIB will be considered. Contractors that exceed these thresholds will be disqualified. LKDSB will obtain this information from the Ontario WSIB during the Evaluation of the RFSQ.

Only Subcontractors that have a 5-year average allowed lost-time injury rate (per 100 workers) below 2 and an allowed no lost-time injury rate (per 100 workers) below 4, as measured by the Ontario WSIB should be considered. Contractors that include subcontractors that exceed these thresholds will be disqualified. LKDSB will obtain this information from the Ontario WSIB during the Evaluation of the RFSQ.

RFSQ Documents will be made available at lkdsb.bidsandtenders.ca on the date the RFSQ is published.
